Community Health Assessments (CHA)

The Community Health Assessments (CHA) use quantitative and qualitative methods to collect and examine health status indicators and provide an understanding of health in a community. The CHA guides the development and implementation of a Community Health Improvement Plan (CHIP) by justifying how and where resources should be allocated to best meet community needs.

Community Health Status Assessment Report                                                                                                                                     This assessment identifies priority community health and quality of life issues.
                                                                                                                   

Community Themes and Strengths Assessment Report                                                                                                                     This assessment results in a strong understanding of community issues and concerns, perceptions about quality of life, and community assets.

Forces of Change Assessment Report                                                                                                                                                 This assessment is aimed at identifying forces—such as trends, factors, or events—that are or will be influencing the health and quality of life of the community and the local public health system.

Local Public Health System Assessment Report                                                                                                                                 This assessment answers the questions, "What are the components, activities, competencies, and capacities of our local public health system?" and "How are the Essential Services being provided to our community?"
