The Lake County Health Department/Community Health Center continually seeks to improve the quality and the efficiency of the health services provided. This is accomplished through Continuous Quality Improvement (CQI) and by monitoring and measurement of performance outcomes.

Lake County Health Department/Community Health Center continually seeks to improve the effectiveness and efficiency of services provided to ensure the health and well-being of those in Lake County. Quality improvement is the process by which this is accomplished and can be described as a deliberate and defined process focused on activities that are responsive to the communities’ needs and improving population health. As an agency we strive to achieve measurable improvements in the efficiency, effectiveness, performance, accountability, outcomes and other indicators in services or processes which achieve equity and improve the health of the community.

The Lake County Health Department and Community Health Center (LCHD/CHC) received national accreditation through the Public Health Accreditation Board (PHAB) on March 11, 2015. Lake County was the 10th of 97 certified local health departments in Illinois and among the first 100 of more than 3,000 health departments nationwide to receive accreditation through PHAB, the independent organization that administers the national public health accreditation program. With accreditation, LCHD/CHC is demonstrating increased accountability and credibility to public, funders, elected officials and partner organizations with which they work. The national accreditation program, jointly supported by the Centers for Disease Control and Prevention and the Robert Wood Johnson Foundation, sets standards against which the nation’s more than 3,000 governmental public health departments can continuously improve the quality of their services and performance. To receive accreditation, a health department must undergo a rigorous, multi-faceted, peer reviewed assessment process to ensure it meets or exceeds a set of quality standards and measures.

Performance Management System
The Performance Management System is utilized to monitor the quality of performance of public health processes, programs, interventions, and other activities. This system helps to effectively and efficiently improve the health of the population.

Joint Commission accreditation and certification is recognized nationwide as a symbol of quality that reflects an organization’s commitment to meeting certain performance standards. The Lake County Community Health Center has received Joint Commission Accreditation.

Quality Improvement
Quality Improvement in Public Health is the use of a deliberate and defined improvement process which is focused on activities that are responsive to community needs and improving population health. It refers to a continuous and ongoing effort to achieve measurable improvements in the efficiency, effectiveness, performance, accountability, outcomes, and other indicators of quality in services or processes which achieve equity and improve the health of the community.1
